I understand that:
  1. U.S. FUTSAL may deny certification to any person who has been convicted of a felony, crime of violence or a crime against a person.
  2. In applying for a U.S. FUTSAL position, the information which I have furnished on this form is subject to verification, which may include a criminal history check.
  3. Physical, mental, and verbal abuse of any of the participants, coaches, managers, employees, referees, volunteers involved in our sponsored activities is not permitted.
  4. Inappropriate touching of any kind is forbidden.
  5. Coaches are responsible to only roster properly registered players.
  6. Coaches should not socialize with the participants outside of the sponsored activities of the organization.
  7. Coaches should never ride alone with a child or participant in the car.
  8. This disclosure statement must be updated every year.

  1. All the information entered on this registration is correct.
  2. I have no physical illness or impairments which will make participating in Futsal/5-a-side related activities dangerous to me.
